Specialized,
long term service coordination is provided to Bogard class
members residing in Madison and St. Clair Counties of Illinois.
Services are provided regardless of the living arrangement or need for
Specialized Services.
Bogard
class members are all persons 18 years of age or older with a
developmental disability, who, on or after March 23, 1986, resided in
an Intermediate Care Facility (ICF) or Skilled Nursing Facility (SNF)
in Illinois as a Medicaid recipient for a period of more than 120
days. No person first admitted to an ICF or SNF on or after April 1,
1994, will be a member of that class.
