Service Coordination - St. Clair County Illinois 

Service Coordination provides an ongoing goal-oriented process for securing a range of services needed by individuals with developmental disabilities and their families.  Services are designed to assure accessibility, availability, continuity of care, accountability, and comprehensiveness to maximize the individual's potential for independence, productivity and community integration.

Service Coordination Includes:  

  • Identification & Outreach

  • Assessment of individual needs

  • Planning & developing an individual service plan

  • Monitoring & Follow-up of services offered

  • Advocacy on behalf of the client & family

  • Service Choice & Linkage to community service providers offering:

    • Respite

    • Day Programs

    • Residential

    • Early Intervention

    • Education

    • Vocational/Employment

    • Counseling

    • Family Support
